• When completing the section asking you to “Please list your major complaints in order of severity”, the first complaint corresponds to Complaint # 1, the second complaint corresponds to Complaint # 2, and the third complaint corresponds to Complaint # 3. If you have more than 3 complaints listed, you may ask the receptionist for an additional form. Thoroughly completed forms will assist the doctor in understanding your condition.

• If you have any special diagnostic studies such as recent x-rays, MRI, CAT Scan, or blood work, please bring those to your initial visit to facilitate your evaluation, or contact our receptionist for instructions on requesting those records from other providers.
